2011-2012 MEDICAL EXEMPTION FORM to request that student be granted a medical exemption from the statewide and accountability program for students in the regular assessment program (Note: a student's handicapping condition, if any, cannot be the basis for a medical exemption)

On-Demand Writing can be extended to the appropriate number of pages using the same extended-response template. Margins, font size and spacing are locked down. Remove the sample text and type in question number and the answer. As in the past, students will have to stop typing at the point where the line goes below the set margin for each sample. Only the text within the answer box will be scored.
